Policies and fees
Cancellations must be made by phone by 9 a.m. the day of your appointment.
Cancellations made after that time, or failing to show for an appointment, are considered missed appointments and an office fee of $50 is charged to your account.
If time is available, you may reschedule an appointment within the same calendar week, and the office fee will be waived if the appointment is kept. If that appointment is missed, however, you will be charged for 2 appointments.

If at all possible, please call and let me know if you are running late. If I do not hear from you by 15 minutes after the scheduled appointment time, it will be considered a missed appointment.
Your participation is important to the success of your treatment. Please discuss with me if there are circumstances interfering with your ability to attend or fully participate in therapy.
Group Requirements
Because therapy groups are difficult to sustain logistially and financially, there are additional requirements to group participation.
Each group has a minimum attendance requirement. Participants must commit to attending a minimum of 8 to 12 weeks depending on the group.
Participants discontinuing early will be charged a portion of the fees for the remaining group sessions.
There is an additional $50 admission fee for all groups except for the psychoeducational group, which has its own fee structure.

Group Therapy $50 admission fee

Psychoeducational groups
Consultations $100 per hour on site plus $1 per mile
Supervision $60 per hour or fee sharing arrangement
Please consult with your insurance provider concerning coverage. Deductibles must be met before insurance will pay. Copays and Co-insurance fees are expected at time of service. Regardless of expected insurance coverage, you are ultimately responsible for payment of services. A credit card guarantee is required to initiate services.
